The undersigned (hereafter, the 'Customer') acknowledges and agrees to the following terms regarding services provided by SALON URUKU (hereafter, the 'Salon').

1. **Services Covered**
  Services may include, but are not limited to: waxing and hair removal, facials, hair coloring, chemical treatments, texture services, permanents, nail services, pedicures, and hair cutting. The Customer understands that all such services carry inherent risks.

2. **Assumption of Risk**
  The Customer voluntarily assumes all risks associated with the services provided, including but not limited to: temporary or permanent hair damage, skin irritation, allergic reactions, chemical sensitivity, burns, cuts, or unsatisfactory cosmetic results. The Customer accepts full responsibility for any personal injury, property damage, or dissatisfaction arising from services received.

3. **Waiver and Release**
  The Customer hereby releases, waives, and forever discharges the Salon and its owners, employees, contractors, agents, successors, and assigns from any and all claims, demands, causes of action, damages, losses, or liabilities of any kind, whether known or unknown, arising out of or related to services received. This waiver does **not** apply to acts of gross negligence or intentional misconduct.

4. **Medical Conditions and Disclosure**
  The Customer affirms that they have disclosed all relevant medical or skin conditions, allergies, sensitivities, medications, or other factors that could impact the services provided. The Customer agrees to notify the Salon prior to future services if such conditions change.

5. **No Guarantees**
  The Salon makes no guarantees regarding results, as individual outcomes may vary based on hair type, skin condition, prior treatments, and other factors outside the Salon’s control.

6. **Indemnification**
  The Customer agrees to indemnify and hold harmless the Salon from any claims or expenses (including attorney fees) incurred due to the Customer’s actions, omissions, or failure to follow aftercare instructions.

7. **Binding Agreement**
  This Waiver and Release constitutes the entire agreement between the parties and supersedes all prior oral or written statements. No modification is valid unless agreed to in writing by both the Customer and the Salon.

8. **Continuing Effect**
  This agreement remains in full force and effect for all future services rendered unless revoked in writing.

9. **Acknowledgment**
  The Customer affirms that they are at least 18 years old, have read and understood this agreement, and sign voluntarily without pressure, duress, or promises outside what is written here.
